Notice on Application for Reader’s Card of Zhejiang Library

Who can apply: Chinese citizens age 12 and above and foreigners.

Documents required: valid documents such as ID card (including Hong Kong, Macao, and Taiwan), passport, home-visit permit for Taiwanese compatriots, and military ID card; students age 12 to 16 who have no ID card can apply with residence booklet.

Application Procedures:

1.Manual Application

①Please get the application form with the above valid documents and fill it out according to your actual condition (non-local and foreign readers are required to provide the address in Hangzhou and telephone number);

②Select functions according to your needs;

③Process application procedures at corresponding window and pay application fees (first-time application charges no processing fee);

④Please keep your invoice, certificate of deposit and valid documents properly after the application.

2. Self-help Application

①Please select Apply for Reader’s Card on self-service machine with your ID card and provide relevant information;

②Select functions according to your needs;

③Pay application fees (cash only);

④Please keep your reader’s card and valid documents properly after application.

Add functions:

1. Manual Application: reader’s card holders can add functions according to their new needs, and you can use newly-added functions after filling out Function Adding Application Form with your ID card and reader’s card and pay fees for the new function and the application is processed by Card Application Center. Please keep your reader’s card and valid documents properly after application.

2. Self-help Application: Select upgrading reader’s card on self-service machine with your reader’s card, then enter your password for reader’s card (default password is the last six digits of ID card number) and confirm your information. Select the functions you need, then pay upgrading fees (cash only). Please keep your reader’s card and valid documents properly after application.

Returning deposit:

You can apply for disenabling borrowing function at Card Application Center of our library at any time. When returning deposit, you must return all books you’ve borrowed and pay off arrearages and process deposit returning procedures with your reader’s card, ID card and deposit certificate at Card Application Center of Zhejiang Library, but other expenses cannot be returned.

Report loss:

If your reader’s card is lost, please register and process reporting procedures at our Card Application Center with your ID card; if you cancel the reporting when you find the card reported missing, functions of original card will be enabled for free; if you’ve got a replacement before canceling, cancelling reporting loss will not be processed; Our staff have the right to confiscate the reader’s card which is reported missing and take necessary measures.

Replacement:

After reporting the missing of reader’s card, you can get a replacement, and functions and term of validity of the card reported missing will be recovered, and you need to pay 15 yuan as processing fee;

Notes: 1. A reader can only apply for one IC card with functions of reader’s card’s Zhejiang Library.

2.Reader’s card doesn’t offer downgrading function.

3.If your reader’s card is found lost, please report loss immediately at Card Application Center; losers shall be responsible for all consequences caused by the lost before reporting loss.

4.If the card cannot be used because reader keeps it inappropriately, the reader need to pay 15 yuan as processing fee for a replacement.
